Your Role:
- Perform Comprehensive Nutrition Assessments and interventions
- Deliver Medical Nutrition Therapy (MNT) in alignment with CMS and AHCA standards
- Complete charting for initials, quarterlies, annuals, post-hospitalizations, High Risk and significant changes
- Collaborate with interdisciplinary teams and participate in IDT meetings
- Conduct audits and lead QAPI initiatives
- Offer telehealth consults and document using EMR systems
- Lead SOC
Qualifications:
- Florida-licensed Registered Dietitian (or RD-Eligible with verification)
- Experience in long-term care preferred not required - willing to train
- Team-oriented with strong communication and documentation skills
- Passionate about patient-centered, evidence-based care
